What Is Ibogaine?
Ibogaine is a naturally occurring psychoactive alkaloid found in the root bark of Tabernanthe iboga, a shrub native to the rainforests of Central West Africa — primarily Gabon, Cameroon, and the Congo Basin. For centuries, the Bwiti people of Gabon have used iboga in sacred initiation ceremonies, consuming large doses to facilitate profound spiritual encounters with ancestors, the self, and the cosmos.
In the 1960s, Howard Lotsof — a young heroin addict — accidentally discovered that a single ibogaine experience had eliminated his withdrawal symptoms and his desire to use opioids. He spent the rest of his life advocating for its medical recognition. Today, ibogaine is the subject of serious clinical research at institutions including Stanford University, the University of California San Francisco, and the Multidisciplinary Association for Psychedelic Studies (MAPS).
At Pō a Ao, we understand ibogaine through two lenses simultaneously: the Western scientific lens of neuropharmacology and clinical outcomes, and the indigenous lens of sacred plant medicine that deserves deep respect and careful ceremony.
The Neuroscience
Ibogaine is pharmacologically unlike any other substance. It acts simultaneously on multiple receptor systems in the brain — a complexity that researchers are still working to fully understand.
Its most significant action is on the opioid receptor system. Ibogaine binds to mu and kappa opioid receptors, effectively resetting the neurological pathways that have been hijacked by opioid dependence. This is why people who complete an ibogaine session often report that their withdrawal symptoms — which would normally last days or weeks — disappear within hours.
Ibogaine also inhibits the reuptake of dopamine, serotonin, and norepinephrine, flooding the brain with the neurotransmitters that addiction depletes. It blocks NMDA glutamate receptors, which plays a role in its dissociative and visionary effects. And it binds to sigma-2 receptors, which are involved in memory consolidation and neuroplasticity.
Perhaps most importantly, ibogaine is metabolized in the body into noribogaine — a compound that remains active for weeks after the experience and continues to support neuroplasticity, the brain's ability to form new connections and break old patterns. This extended window is one reason why ibogaine's effects on addiction can be so durable.
A landmark 2023 Stanford study of 30 Special Operations veterans found that ibogaine treatment produced dramatic reductions in PTSD symptoms (88%), depression (87%), and anxiety (81%) — with effects sustained at one-month follow-up.
What to Expect
An ibogaine experience unfolds in two distinct phases over approximately 24–36 hours.
The first phase — the visionary or oneiric phase — begins 1–3 hours after ingestion and lasts 4–8 hours. During this phase, guests typically experience vivid, dreamlike visions. These are not hallucinations in the recreational sense; they are more like a waking dream in which the unconscious mind presents memories, patterns, and truths for examination. Many people describe meeting deceased loved ones, revisiting childhood memories, or receiving clear insights about the root causes of their suffering. The experience can be intense and emotionally confronting — but our medical team and guides are present throughout.
The second phase — the introspective or cognitive phase — follows the visions and can last 8–20 hours. During this time, guests feel deeply reflective, often processing what they witnessed in the first phase. Sleep is difficult during this period. The mind is extraordinarily active, making connections and integrating insights. Many guests describe this as the most mentally clear they have ever felt.
After the experience, guests typically feel a profound sense of calm, clarity, and openness. The neuroplasticity window that follows — lasting several weeks — is the critical integration period during which new patterns can be established.
Conditions It Addresses
Ibogaine has shown the most dramatic results in the treatment of opioid use disorder, including addiction to heroin, fentanyl, oxycodone, and methadone. Multiple studies have documented 50–80% reductions in withdrawal symptoms and significant reductions in cravings at 30-day follow-up.
Beyond opioid addiction, ibogaine has shown promising results for alcohol use disorder, stimulant addiction (cocaine, methamphetamine), treatment-resistant depression, PTSD and complex trauma, traumatic brain injury (TBI) — particularly in veterans — and existential distress.
It is important to understand that ibogaine is not a cure. It is a powerful catalyst — an interruption of deeply ingrained patterns that creates a window of opportunity. What you do with that window — through integration, lifestyle change, and ongoing support — determines the durability of your healing.

Medical Contraindications
Ibogaine is not appropriate for everyone. The following conditions are absolute or relative contraindications. Every guest at Pō a Ao undergoes a full medical screening including EKG before any ibogaine program is approved.
- Cardiac arrhythmias or prolonged QTc interval (EKG required before treatment)
- Severe liver disease or hepatic impairment
- Active psychosis or schizophrenia
- Current use of certain medications (SSRIs, MAOIs, opioids — tapering required)
- Pregnancy or breastfeeding
- Severe kidney disease
- Uncontrolled hypertension
